Behavioral Health Insurance & Sliding Fee Scale
We believe everyone should have access to quality behavioral health care.
MFS maintains a Sliding Fee Scale and no one is denied services due to an inability to pay. Clients who are uninsured, underinsured or have difficulty paying for services are eligible to receive services at a reduced fee or no charge.

Metropolitan Family Services is currently a provider for Medicaid, Medicaid MCO Plans, Medicare, D-SNP plans and several commercial insurance plans.

Metropolitan Family Services takes Commercial Insurance/PPO plans as an out of network provider after confirmation from the insurance plan.
Please call 312-986-4084 and ask to speak with a Registration & Billing Specialist who can help you determine your financial responsibility, navigate your insurance coverage, apply for our Sliding Fee Scale, request a reduction or waiver, or appeal a fee decision. MFS may use self-attestation or provisional eligibility. You will not be denied, delayed, suspended, or terminated from services because you cannot afford to pay.
