PROGRAMA PADRES COLABORADORES
Parents as Partners provides weekly home visiting services to parents between the ages of 12 and 30, whose family is expecting or has a child birth to 5 years. As a client in our program, you will receive information on your child’s development as they grow and on various topics including nutrition, safety, health and so much more.
Our qualified staff work with clients to locate resources that ensure the health and wellness of the family. This includes helping families locate employment opportunities, childcare, healthcare professionals, and counselors.
¿Cómo puedo reunir los requisitos?
- Clients must have Medicaid. If uninsured, clients must be eligible to apply for Medicaid
- Clients must be between 12 and 30 years old
- Clients must be pregnant and/or parenting a child under 5 years old
- Clients must live in one of the following communities: Archer Heights, Ashburn, Brighton Park, Chicago Lawn, Gage Park, Marquette Park, New City (Back of the Yards), West Elsdon, or West Lawn
